Case Manager Long-term Care - Delaware

This role serves as the primary point of contact for members to coordinate care needs across various service delivery systems and community supports. It involves conducting in-home and nursing facility visits, assessing complex health and social care needs, and developing individualized care plans for individuals enrolled in long-term services and supports programs. The incumbent collaborates with interdisciplinary teams to ensure safe, cost-effective care transitions and addresses physical, behavioral, and psychosocial needs.
