About this role
The Northern Outreach Case Manager at Launch Housing delivers outreach-based support to people who are experiencing or at risk of homelessness across northern Melbourne (City of Yarra, City of Merri-Bek and City of Darebin). The role focuses on building therapeutic relationships to improve clients' quality of life and social inclusion and supporting housing stability and homelessness prevention. This is a maximum-term full-time position based in Collingwood until January 2027 with a Melbourne-based nonprofit committed to ending homelessness.
