Case Manager (Paraprofessional)

The Case Manager (Paraprofessional) provides direct client support in both Psychiatric Emergency Services and the Crisis Stabilization Unit, handling intake, triage, ongoing monitoring, group facilitation, and discharge coordination. The role works closely with multidisciplinary teams to assess needs, deliver trauma‑informed care, and connect clients to community resources in a high‑acuity crisis stabilization environment.
