Case Manager- Peer Support

The Case Manager-Peer Support provides trauma-informed case management, peer mentorship, and integrated co-occurring services to adults with serious mental illness within the Veteran’s Wellness Program. This full-time role involves managing a caseload, facilitating crisis care, building daily living skills, and coordinating community resources to promote client recovery and wellness. The position requires collaboration with clinical teams, community outreach, and maintaining accurate documentation while adhering to trauma-informed care principles.
