Case Manager, Permanent Supportive Housing

The Case Manager, Permanent Supportive Housing helps chronically homeless individuals engage in Intensive Case Management to achieve sustainable housing and connect with mental health and other services. This in-person, field-based role requires establishing trusting relationships with clients using a trauma-informed approach while managing case notes and coordinating housing stabilization services. The position involves regular travel throughout Marin County to support clients in various community and residential settings.
