Case Manager - Prototypes- Pomona Women's Center

As a Case Manager, you will directly support clients in overcoming substance use disorders and mental health challenges through in-person coordination of care within residential, outpatient, and community settings. You will assist with intake assessments, benefit applications, resource linkage, and treatment plan monitoring while collaborating with a multidisciplinary team. A valid driver's license and access to reliable transportation are required, along with specific certifications or a relevant bachelor's degree.
