About this role
The Case Manager, QMHA works within Cascadia’s Integrated Team Based Care program at the Plaza Health Center in SE Portland, providing community-based mental health support for adults. The position supports Cascadia’s whole-health model by coordinating care across mental health, addiction services, primary care, and housing while promoting trauma-informed, person-centered, and recovery-oriented care. This is an onsite, union-represented role with regular daytime hours.
