About this role
The Case Manager - Reentry supports individuals transitioning from closed custody by coordinating person-centered reentry services at Cherry Health’s residential programs. The role focuses on ensuring continuity of care and facilitating connections to health, behavioral health, housing, education, and employment resources to support successful community reintegration. This is a full-time, onsite position at the Community Treatment Center in Detroit.
