About this role
Hospital for Special Surgery (HSS) is hiring a full-time Case Manager to support patients across the continuum of care at a top-ranked orthopedic and rheumatology hospital in New York City. The role focuses on managing patient needs from inpatient stay through discharge and transition to post-acute providers, working closely with patients, families, and clinical stakeholders. This position is a day shift Monday–Friday (8am–4pm) with occasional weekend coverage (one weekend shift per quarter).
