Case Manager RN

The Case Manager RN leads the rehabilitation neighborhood and serves on the community's leadership team, guiding residents through successful transitions from skilled nursing care back to their home environment. This role involves collaborating with care providers to align services with resident goals, facilitating interdisciplinary communication, and supporting residents in managing their own care. The position also includes assisting the Director of Nursing and participating in an on-call rotation.
