Case Manager (RN)

The Case Manager leads care coordination across hospital levels of care, completing assessments and transition care planning tailored to patient and family needs. This role assists physicians and finance in managing resource utilization through medical necessity criteria and conducts Interqual utilization reviews. The position also involves building provider networks, leading morning huddles, and conducting patient interviews to determine social determinants of health and discharge disposition.
