About this role

The Case Manager, Social Work, PRN, BSW or MSW organizes and expedites treatment plans for medically complex patients, addressing difficult social issues related to hospital progression of care. The role involves identifying discharge needs, developing discharge plans, and coordinating care with clinical partners and community resources.
