Case Manager-Street Outreach

Newport Mental Health is seeking a Case Manager for its Street Outreach department to provide outreach, navigation, and supportive services to individuals experiencing homelessness due to mental health or substance use disorders. The role involves developing strategies to engage clients in shelters and housing authorities, linking them to bridge and permanent housing programs, and maintaining professional relationships with community partners. This position requires a minimum of two years of experience providing street-based services to highly vulnerable populations and an Associate's Degree in a related Human Services field.
