About this role
Case Manager for the Transition Age Youth (TAY) Program providing case management services to youth ages 18–25 with serious and persistent mental health issues. The role is based in San Leandro and supports clients and families through community-based, trauma-informed, and culturally responsive services. The position is in-person, Monday–Friday AM shift, and focuses on helping youth transition to adult life.
