About this role

The Case Manager will work directly with families in Lighthouse's Emergency Family Shelter, providing housing case management and connecting households to resources to obtain and sustain housing. The role involves intake screening, maintaining a caseload, conducting assessments, and facilitating the transition to permanent housing while coordinating with internal and external partners.

Responsibilities

Perform client screenings and intakes using HMIS
Maintain a caseload of 15‑20 households and develop service plans
Coordinate nightly shelter check‑ins and communicate client updates
Conduct housing searches, secure documentation, and complete HQS inspections
Facilitate inter‑agency collaborations and discharge planning for housing transitions
