Case Manager

This role utilizes an Integrated Case Management Model to ensure high-quality, evidence-based care and efficient treatment for all patients. The Case Manager facilitates utilization review, care coordination, advocacy, and discharge planning while monitoring cases for appropriateness of continued stay and level of care. The position involves collaborating with physicians, external payers, and multidisciplinary teams to optimize patient outcomes and resolve barriers to discharge.
