Case Manager

The Care Manager provides services to individuals with severe and persistent mental illness, serious emotional disturbance, and/or intellectual or developmental disabilities, ensuring they receive necessary supports for community inclusion and independence. This role involves advocacy, service coordination, linkage to supports, and ongoing monitoring to ensure quality and continuity of care. Core functions include service planning, assessment, coordination, and documentation to promote empowerment and self-determination.
