Case Manager

The Case Manager is responsible for implementing and maintaining program requirements at the facility, including instituting individual program plans, orientation program plans, and conducting regular reviews. This role involves assessing resident needs, providing program orientation and life skills training, and assisting residents in meeting their established goals. The position also requires maintaining accurate case files, documenting activities, and supporting the Director with community relations and grievance investigations.
