Case Manager

The Case Manager partners with families referred by the Department of Human Services to strengthen parenting practices, enhance protective capacities, and create a nurturing home environment that promotes child safety, well-being, and permanency. This role involves meeting visitation requirements, completing safety assessments and service plans, and collaborating with service providers and the court system to remove barriers to permanency. Success is demonstrated through measurable improvements in child safety, family stability, and permanency outcomes.
