Case Manager

This role provides professional case management and coordination services for individuals and families enrolled in the Shelter Plus Care permanent supportive housing program. The position assists households with housing navigation, including rent calculations, lease agreements, and landlord recruitment, while developing person-centered service plans. The Case Manager works under general supervision to link participants with supportive services and monitor their progress through home visits and phone contacts.
