Case Manager

The Case Manager provides ongoing, documented case management to adults enrolled in a low-threshold program offering harm reduction, health promotion, care coordination, and recovery support for people who use drugs or are at risk of overdose. The role involves conducting assessments, developing service plans, coordinating referrals, and helping participants work toward their goals using participant-centered, trauma-informed, and culturally responsive approaches. This position maintains an individualized case management relationship to support access to medical care, behavioral health services, housing, and other essential health and social supports.
