Case Manager

The Case Manager provides direct case management and support services to a caseload of 20 formerly chronically homeless residents, assisting them in obtaining benefits, housing linkages, and health services. This role involves collaboratively developing housing stabilization plans, conducting apartment checks, and linking residents to community resources to promote self-sufficiency and safety. The position requires flexibility, strong time management, and the ability to work both independently and within a team environment.
