Case Manager

Penny Lane’s Permanent Supportive Housing Program is seeking a Case Manager to provide direct support services to a caseload of 20 formerly chronically homeless residents. The role involves assisting participants with obtaining essential services such as benefits, housing linkages, and health treatments while developing strategies for housing stabilization and self-sufficiency. This position requires flexibility to work both in the office and within residents' apartments to ensure safety and maintenance.
