Case Manager

This position provides in-home support to families to promote positive parent-child relationships and offers case management and linkage to community resources. The role involves conducting home visits, advocating for families to receive entitlements and benefits, and assisting with connections to housing and other services. The incumbent will also provide supportive casework, complete required documentation, and participate in team meetings and training.
