Case Manager

This role is responsible for assessing the psychosocial needs of low-income older adults and coordinating support services to ensure quality care. The position involves developing and monitoring client-centered Community Living Plans, managing relationships with clients and community providers, and documenting all case management activities. The ideal candidate will have experience working with the frail elderly and a strong understanding of healthcare entitlement programs.
