Case Manager

This role provides emotional support, crisis intervention, and counseling to patients and families dealing with hospitalization, illness, disability, or trauma. The Case Manager coordinates discharge planning, completes psychosocial assessments, and assists in the development and re-evaluation of treatment plans as part of a multidisciplinary team. The position requires maintaining clinical records and ensuring necessary documentation is completed throughout the client's length of stay.
