Case Manager

This role assesses the eligibility and needs of individuals and families experiencing homelessness to facilitate their transition to transitional or permanent housing. The position involves developing case management plans, providing advocacy and mediation with landlords, and assisting clients in securing housing and achieving self-sufficiency. The Case Manager also maintains accurate records, collaborates with community resources, and ensures adherence to personal budgets and service goals.
