Case Manager

This role is responsible for managing and coordinating patient care across the continuum of illness, ensuring effective utilization of healthcare resources and achieving optimal clinical outcomes. The position involves leading interdisciplinary teams, conducting daily chart and utilization reviews, and facilitating efficient discharge planning to maximize patient health. The Case Manager also works to resolve care barriers, track clinical pathway effectiveness, and represent the department in community outreach efforts.
