Coordinates utilization management, resource management, and discharge planning to provide integrated care and prevent unnecessary admissions or readmissions. Oversees acute patient populations across the care continuum, focusing on coordinated care for patients with complex health needs. Provides discharge planning coordination and intervention for high-risk patients presenting to the Emergency Department.
