Case Manager

The Case Manager provides support by coordinating a variety of services for consumers, including identifying needs, advocating, linking resources, and providing crisis services. The role involves conducting psychosocial assessments, developing treatment plans, and collaborating with treatment teams to ensure quality care. Case Managers also engage with consumers and families, maintain case records, and facilitate support groups and community resources.
