Case Manager

This position provides moderately complex case management work, developing and maintaining long-term contact with individuals, families, and service providers to address medical, social, and educational needs. The role involves creating and implementing service plans, coordinating provider activities, and delivering psychosocial rehabilitation and crisis intervention services under general clinical supervision. The position requires a strong commitment to trauma-informed care and the recovery model while ensuring compliance with state regulations and center policies.
