Case Manager

This role is responsible for implementing and monitoring housing and social service plans for families assigned to the Essence Hotel Shelter. The position involves conducting family intakes, designing social service plans, and advocating for clients to secure entitlements and permanent housing. The Case Manager also coordinates housing searches, makes referrals for additional services, and collaborates with various agencies and internal teams to support client needs.
