Case Manager

The Case Manager provides outreach and housing placement services to homeless individuals in the North Shore and other areas of Staten Island. The role involves conducting assessments, facilitating referrals for medical and social services, and counseling clients toward interim, transitional, and permanent housing placements. The position requires responding to community calls, managing client paperwork, and driving clients to necessary facilities using an outreach van.
