Case Manager

The Case Manager works within the Outreach and Housing Placement Program, providing outreach to homeless individuals in the North Shore and other areas of Staten Island. The role involves assessing client needs, making appropriate referrals for services, and counseling clients toward interim, transitional, and permanent housing placements. The position requires responding to community calls, completing necessary paperwork, and driving clients to essential services as needed.
