Case Manager

This role focuses on providing psychosocial case management services to Montana Big Sky Waiver Medicaid beneficiaries who are elderly or physically disabled. The position involves conducting assessments, coordinating community resources, and supporting members' housing, financial stability, and long-term care needs. The goal is to improve quality of life and provide emotional support within a multi-disciplinary Care Management Team.
