Case Manager

The Case Manager provides strength-based, person-centered case management to individuals and families experiencing homelessness or housing instability. This position works with participants to identify needs, navigate housing and community resources, access available assistance, and develop goals that support housing stability and self-sufficiency. The role involves completing participant assessments, maintaining documentation, and collaborating with community partners to help individuals and families obtain and maintain stable housing.
