Case Manager

The Case Manager supports the department by providing case management to different units within the facility and coordinating care across multiple providers to ensure clients receive support identified within their individualized treatment plan. This role is responsible for overall discharge and aftercare planning, maintaining ongoing communication with the treatment team, and participating in quality care monitoring. The position requires collaboration with clients, families, and community resources to implement effective service plans and ensure all prescribed services are delivered.
