Case Manager

The Case Manager coordinates daily schedules and treatment-related activities for youth residents at The Centers' H.O.P.E. Campus, serving as a liaison between the facility and external agencies such as child welfare. This role ensures residents receive consistent, individualized care by managing appointments, coordinating visits, and transporting clients as needed. The position supports the organization's mission to provide a safe, trauma-informed residential environment for youth healing and growth.
