Case Manager

This role provides guidance, support, and case management for the Fresenius Kidney Care renal patient population, acting as a subject matter expert for patients, dialysis staff, and physicians. The position involves coordinating care, delivering education, and managing program implementation through telephonic and occasional on-site interactions. The Case Manager ensures high-quality continuity of services and cost-effective delivery while adhering to regulatory and company policies.
