Case Manager

Colonial Behavioral Health is seeking a Case Manager to join its Adult Mental Health department, where you will work directly with adults experiencing serious mental illness and/or substance use disorders. In this role, you will help individuals identify their goals, navigate challenges, and connect with essential resources and services to support their recovery and well-being. The position involves developing individualized service plans, coordinating care, and maintaining accurate documentation while collaborating with clients and their support networks.
