Case Manager

This fully onsite, client-facing role provides short-term case management services to seniors and adults with disabilities, including those experiencing homelessness, behavioral health conditions, and substance use challenges. The position involves conducting assessments, developing care plans, facilitating support groups, and collaborating with partner agencies to secure appropriate housing and treatment for clients. Employees are expected to maintain active caseloads, navigate centralized intake systems, and adhere to strict safety and documentation protocols.
