Case Manager

This role involves functioning as part of a multidisciplinary team to conduct psychosocial assessments and formulate service coordination plans for patients and families. The position requires acting as a patient advocate to negotiate necessary services and facilitate referrals throughout the network and community. Additionally, the role includes developing treatment plans in accordance with grantor requirements and ensuring the quality of case management services through continuous evaluation.
