Case Manager

The Case Manager is responsible for completing and submitting assessments and service plans to compile comprehensive case files within the Southwest Key Programs web-based system. This role involves maintaining organized client files, establishing therapeutic relationships, and providing crisis intervention and advocacy for clients and their families. The position requires flexibility to work evenings, weekends, and on-call rotations to facilitate program services.
