Case Manager

The Case Manager provides residential and long-term support services to homeless and at-risk adults transitioning into permanent housing, managing a caseload of approximately 20 to 25 individuals. The role involves conducting assessments, developing service plans, and coordinating with community providers to support tenants' housing, employment, and wellness goals. Case Managers utilize evidence-based models and trauma-informed interventions to help clients overcome barriers and achieve stability.
