Case Manager

The Case Manager manages and coordinates comprehensive services to support consumers in safely living in a Home and Community Based Setting under the supervision of the Director of Home and Community Based Services. The role involves assessing client needs, ensuring compliance with PASSPORT program expectations, and maintaining proficiency in relevant software and documentation. This position requires effective communication, travel within a four-county service area, and adherence to safety and regulatory standards.
