Case Manager

The Case Manager ensures clients' discharge plans are appropriate, coordinated, and followed through from admission through all levels of care. This role acts as a liaison and resource for alumni, fostering long-term relationships while facilitating a multi-directional discharge plan to support the patient's journey of recovery and well-being. The position involves coordinating with clinical and medical teams, managing documentation, and linking clients to community resources.
