Case Manager

Southwest Behavioral & Health Services is seeking a Case Manager to join their Buckeye Outpatient team, providing direct services in the home, community, or office to support consumers in achieving their treatment goals and improving their level of functioning. The role involves conducting assessments, developing service plans, coordinating care, and providing crisis intervention to assist consumers and their families in retaining skills and remaining in the community. This position requires a Bachelor's degree, behavioral health experience, and a valid Arizona driver's license.
