Case Manager

The Case Manager coordinates services, documentation, and communication for youth and families at West Central Ohio Academy, working under licensed mental health supervision to support assessment, treatment planning, and discharge preparation. This role involves collaborating with multidisciplinary teams, families, and community partners to ensure comprehensive care and successful transitions for at-risk adolescent girls.
