Case Manager

The Case Manager provides comprehensive case management and service coordination to eligible veterans, service members, and their families participating in the Sergeant Fox Suicide Prevention Grant Program. The role involves assessing participant needs, developing individualized service plans, coordinating referrals, and supporting access to behavioral health, housing, employment, and other community-based resources. This position focuses on suicide prevention, safety monitoring, and collaboration with clinical and community partners to address risk factors and promote wellness.
