Case Manager

Springwell is seeking a Case Manager to join an interdisciplinary team, conducting comprehensive assessments of older adults to determine their needs and eligibility for in-home services. The role involves performing in-person home visits, documenting assessments in an electronic statewide database, and creating care plans. The Case Manager coordinates services with a network of vendor agencies and communicates with family members, staff, and community professionals.
